  • Emotional Well-being

Our behavioral health component considers a patient’s entire physical and mental well-being when addressing health issues.

During a patient visit at NorthShore Health Centers, a Family Practice doctor or nurse practitioner will review your medical history, vital signs, and other important readings and information collected by our professional nursing staff. If necessary, further tests will be ordered to determine a diagnosis. NorthShore Health Centers’ doctors and nurse practitioners – and, when needed, a behavioral health consultant – will then create a treatment plan, which may include medication, dietary changes, minor surgical procedures, or other care as needed. A follow-up visit will also be scheduled. You can rest easy knowing that our state-of-the-art technology makes vital patient information instantly available to our providers at the time of your appointment.

With superior clinical expertise and experienced doctors right in your area, our team is committed to meeting all of your healthcare needs in order to help you live well and feel your best.

Concerned About Diabetes?

Diabetes is a serious disease that can cause heart attack, stroke, blindness, kidney failure, or loss of feet or legs. Prediabetes means your blood glucose (sugar) is higher than normal, but not yet diabetes. If caught early enough, Type 2 diabetes can be delayed or prevented in people with prediabetes through effective lifestyle programs.

Take this test to find out if you are at risk for prediabetes. Schedule an appointment today at 219-763-8112 if you have concerns about your health or risk of diabetes.

Adult Vaccinations

Family Practice doctors and nurse practitioners will also ensure adults are up-to-date on their immunizations. Recommended vaccines may be based on age, health condition or if you will be traveling outside of the company. Use the CDC recommendations below or talk to your doctor or nurse practitioner for additional information.
