Patient Portal

NorthShore Health Centers is proud to offer you access to our Patient Portal, a secure and easy-to-use Internet-based portal which simplifies communication with your Provider. Portal del Paciente disponible en Español.

As of August 12, 2024, NorthShore has upgraded to a new patient portal providing our patients a more quality user experience. All patients must call or come into the clinic to enroll in the new portal or will be automatically enrolled at their next appointment. The old portal will be accessible for 30 days after which patient access will be removed. Medical history will transfer to the new portal, but other information such as messages will not. Please be sure to save any pertinent messages or other information before September 11th.

You can access the Portal from the comforts of your Home, Office or Smartphone in order to:

  • Request prescription refills
  • Access your health information – Patients can also request medical records online.
  • Obtain educational information
  • Message your Provider

  • Request electronic copies of health records
  • View statements and pay online
