NorthShore Health Centers
NorthShore Health Centers is a non-profit committed to providing quality, affordable healthcare for all in Porter, Lake, La Porte, and Jasper Counties. With over 20 locations offering medical, dental, vision, chiropractic, mental health, and medication assistance, including after-hours and urgent care, we serve both scheduled and walk-in patients. Our dedicated team includes primary care doctors, specialists, and support staff focused on addressing your complete health needs.
NorthShore has four dental offices located in Portage, Lake Station (Dr. Fernando Rivera Dental Center), Merrillville, and Hammond, which provide preventative as well as corrective dental care to the communities we serve. NorthShore has recently added Chiropractic services to our Portage, Hammond, Lake Station, and Chesterton clinics to assist patients with spinal or musculoskeletal concerns.
NorthShore is always striving to improve our quality of care. Our facilities employ primary care doctors, obstetricians, midwives, pediatricians, nurse practitioners, prenatal nurses, family support specialists, case managers, patient navigators, and community relations specialists. Our Psychiatrists, Behavioral Health Consultants, and Substance Abuse Counselors work with our Primary Care Providers to ensure that emotional and behavioral issues are addressed as well as physical ones. We address your entire health and well-being. We aim to improve the community's health, one patient at a time.
Other Services
Our clinics offer digital X-rays, ultrasounds, labs, and pharmacies at multiple locations. Healthy Indiana Plan (HIP), Medicaid, and Marketplace enrollment services are available Monday through Friday on both an appointment and a walk-in basis, and Spanish-speaking employees and translation services are at all sites to provide Spanish-speaking patients with interpretation assistance.
NorthShore offers a sliding scale based on income for those with no medical insurance who qualify. Based on yearly household income, patients can pay from a minimum of $25 (0% on the sliding scale) up to $65 (100% on the sliding scale) for an office visit. This sliding scale does not cover lab and other outside fees, however, as those prices are set by third-party providers.
This health center receives HHS funding and has Federal Public Health Services (PHS) deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.
