Family Practice

A primary care provider makes the initial diagnosis and provides continued care. They identify hypertension through blood pressure measurements, assess risk factors, order necessary labs, and begin treatment plans that may include lifestyle changes and initial medications.

Ongoing care includes regularly tracking blood pressure readings, evaluating how you respond to treatment, and make changes to your care plan based on how you are responding to treatment and any new symptoms.

Providers will also provide you with education in simple terms, reviewing your goals and ensuring you understand how to properly manage your blood pressure in between office visits.

Providers prescribe appropriate medications to control your blood pressure, adjust dosages, manage side effects, and collaborate with other members of the care team to improve blood pressure control.

Your primary care provider will also provide routine screenings for hypertension related complications to identify issues early and prevent long-term damage.

Pharmacist

Our pharmacists play a key role in managing hypertension by reviewing medications to ensure appropriate drug choice, dosing, and adherence while identifying interactions.

They monitor and help manage side effects to improve safety and tolerability.

They can also provide education on proper medication use, lifestyle changes and the importance of regular blood pressure monitoring to achieve better blood pressure control.

Behavioral Health

Living with a chronic health condition, such as hypertension, can be stressful and hard on your mental and emotional health. Making sure that you are taking care of every aspect of your health is important. Northshore has a team of mental health professionals that an assist with stress management, depression, anxiety, and support you through lifestyle behavior changes. Encouragement and support from friends and family is important, but if you feel that you need additional help, talk to your family practice provider how an appointment with our behavioral health team can benefit you.

Support Services

Northshore has staff to assist in all areas of care. Care coordinators are available to help with blood pressure education and monitoring, nutrition and healthy lifestyle coaching, follow up scheduling, and support in between appointments. Our Community Resource Advocates and Insurance Navigators are available to help with transportation resources, social services connections and insurance enrollment support.
