If you are uninsured or underinsured, NorthShore Health Centers has trained navigators available to assist you with insurance options and eliminate any barriers to your health and wellness needs.

Medicaid Enrollment

Enrollment assistance is available at the following NorthShore locations: Lake Station, Chesterton, Merrillville, Portage, Crown Point and Hammond. The state of Indiana offers the Medicaid program for low-income families, including adults, children, and pregnant women.

Please bring the following items to your appointment:

• ID
• Social Security Card
• Birth Certificate
• Last 30 days of income

For Medicare Advantage Enrollment also bring:

• List of medications
• List of doctors
• Medicare A/B Card
