Wellness on Wheels
NorthShore Health Centers is bringing healthcare to you! Wellness on Wheels, our state-of-the art mobile clinic, offers quality, affordable primary care services to our vulnerable populations in Lake, Porter, LaPorte and Jasper Counties. Wellness on Wheels is equipped with full exam rooms, a lab and vaccinations allowing us to serve patients experiencing barriers to healthcare including transportation, homelessness, mobility challenges, mental illness and more.
How it works:
As part of our commitment to making healthcare accessible to everyone, NorthShore Health Centers partners with organizations and agencies across Northwest Indiana to bring quality care directly to their clients. Visits through Wellness on Wheels are the same as an appointment at any of our clinics. Patients will need to fill out the required intake paperwork. We will verify insurance or offer our sliding scale fee based on income for patients without insurance. Insurance enrollment assistance will also be available with our Certified Insurance Navigators.
